I, the undersigned, am the parent and/or guardian to the above-named child. I understand that my child will agree to follow the rules and regulations of the instructor and other staff members and I agree to indemnify and save harmless the martial arts school, its owners, officers, and employees (hereafter called sponsors) from all losses caused by accident or injury to my child. I also agree to allow the sponsors to copy, use and publish any photographic portraits, pictures, or video tapes of my child.
